Health Care Survey of Adults with Disabilities in Connecticut

Overview

The purpose of this project was to better understand the challenges people with disabilities face in accessing and receiving health care. The aim of this study was not only to identify necessary components of the medical home concept, but also to highlight specific barriers individuals with disabilities face regarding adequate access to and receiving of health care.

Accomplishments

  • A telephone survey was developed to gather information from adults with disabilities residing in Connecticut on their experiences with the health care system.
  • 88 surveys were conducted and analyzed.
  • Six components to represent the medical home concept were identified: (1) access to health care; (2) usual source of care; (3) personal doctor or nurse; (4) referrals for specialty care; (5) coordinated care; and (6) person-centered care.
  • A data report and presentation were developed and disseminated.
