Health Care Survey of Adults with Disabilities in Connecticut
Overview
The purpose of this project is to better understand the challenges people with disabilities face in accessing and receiving health care. Center staff conducted a telephone survey of adults with disabilities in Connecticut where participants were asked a variety of questions to provide insight regarding their experiences with the health care system. In particular, questions were designed to identify any barriers to medical homes among participants. Adults with disabilities who live in Connecticut were eligible to participate. For purposes of this survey, an adult was considered to be anyone at least 18 years old, and a disability was defined as any physical or mental disability that significantly impacts one or more of the major life activities as defined by the Americans with Disabilities Act. Emails describing the survey were sent to individuals on the UCEDD list-serve, as well as individuals on the Disability Advocacy Collaborative listserve. Allied Community Services included a flyer describing the survey in paychecks that were mailed to employers with disabilities who receive services from the fiscal intermediary. In addition, libraries, provider agencies, and physicians were asked to post the flyer to assist with recruitment. Individuals interested in the survey were asked to call to schedule a time to complete the survey via telephone with a trained interviewer.
The aim of this study was not only to identify necessary components of the medical home concept, but also to highlight specific barriers individuals with disabilities face regarding adequate access to and receiving of health care.

Early Childhood initiatives at the UCEDD focus on evidenced based practices and learning outcomes. We provide training and technical assistance to early interventionists, teachers, therapists, service coordinators, policy makers and families on a variety of areas including social competence, early literacy, early childhood outcomes, service coordination practices, and evidenced based intervention practices.
School Age initiatives at the UCEDD focus on the inclusion of children with disabilities in typical school and community life. We provide training and technical assistance to families, school personnel and community members in a variety of areas including the special education process, inclusive education, adapted curriculum, assistive technology, disability awareness and person-centered planning.
Adult initiatives at the UCEDD focus on including people with disabilities in their communities. We provide training and technical assistance to individuals with disabilities, state agencies, service providers and other community members in a variety of areas including the hiring of personal assistants, accessible transportation, emergency preparedness and response, and health care.
